| United Way Contributions | The Parents Club of Troop 497 directed donation number for the United Way is #7537. Use this number is you want your donation to go the Parents Club of Troop 497. | ||

Family Account Withdrawal | This form must be filled out and given to the Troop treasurer to request funds be withdrawn from the family account for valid expenditures. | |

Check Request | This form must be filled out and given to the Troop treasurer along with receipts to be reimbursed for legitimate, approved, expenditures. This form is available at Troop meetings. | |
| Troop Activity Permission Slip | Troop 497 Parental Consent and Medical Release must be completed for all Scouts before they can participate in the outdoor program. It should be submitted to Mr. Reidler. | ||

Medical Form for Scouts and Adults | Starting 1 January 2010 there is a new consolidated medical form for all Scouts and Scouters. It must be completed annually. An annual physical is required for everyone participating in events lasting 72 continuous hours, e.g., summer camp. You can read the FAQ (Frequently Asked Questions) on the BSA web site. Please submit your completed medical forms to Mr. Reidler. | |

Troop 497 Medication Form | This form must be completed by a parent or guardian for each prescription and non prescription medicine that a Scout is required to take on a regular basis. The form should be put in the same plastic bag with the medication. |
| Climbing Release | This form must be read, signed, and returned to Mr. Charpentier prior to any Scout or Adult participating in Troop sponsored rock climbing or rappelling. This form must be completed annually. | ||
| Local Tour Permit Application | This form is completed by the adult arranging a trip and must be submitted to the Scout Office prior to a trip. | ||
| Driver List | This form can be used to track the drivers for trips. It contains a place for the names of the drivers and the amount of passengers going and returning with each driver. | ||

497 Eagle Scout Schedule Form |
Troop 497 supports and encourages all Scouts to attain the rank of Eagle Scout. An active Scout can attain the rank of Eagle Scout at any time until the age of 18 (based on BSA criteria). The following form must be signed by all Eagle Scout candidates for Troop 497. It details the time schedule required to submit material required to obtain the rank of Eagle Scout. |

This form must be presented to the Troop Committee Chairman in order to schedule an Eagle Project Presentation to the Committee. This form provides information necessary for the Eagle rank in a concise representation easily verified by the Troop Committee. |
